Read below before booking to make sure you're in the right place.

New Patients

Online scheduling is for new patient initial consultations only. If this is your first visit to Synergy Rheumatology and you'd like to schedule an initial consultation, please review the information below before booking.

Before You Book

Verify your insurance

Please confirm we're in-network with your plan before scheduling. Visit our insurance page for a full list of accepted plans. We collect deductibles, copays, and/or estimated coinsurance at the time of your visit.

HMO plans require a referral

If you have an HMO plan, a referral from your primary care physician must be in place before your appointment. We may need to reschedule or cancel appointments if we're unable to verify an active referral prior to your visit.

New patient forms

After booking, we'll send a packet to the email you provide — including a demographic and medical history form, medical records release, a copy of our patient policies, and a form to upload photos of your insurance card. Please complete these at least 24 hours before your visit, especially the insurance upload. This gives us time to verify your coverage so there are no surprises when you arrive.

Your medical records

While we're an independent practice, we have access to records from all major local hospital systems — Scripps, Sharp, and UCSD — as well as Labcorp, Quest, Imaging Healthcare Specialists, and Pacific Imaging/RadNet. If you have records from other providers that may be relevant, bring them to your first visit or let us know and we can request them afterward.
